Botulinum toxin causes the (smooth) muscles around the blood vessels to relax slightly. The result is that more blood enters the cavernous body of the penis. As a result, the penis appears more blood-filled and looks larger both when flaccid and erect. There are also studies that use botulinum toxin to reduce premature ejaculation (ejaculatio praecox).

Basically, botulinum toxin treatment on the penis must be tried out to see to what extent it is good for individual needs. The treatment is a so-called “off-label use”. This means that botulinum toxin is not approved for this purpose. However, the effects and side effects are the same as for use on other parts of the body. The treatment takes about 20 minutes and involves a few injections into the penis from both sides using a very, very small needle. It is usually well tolerated and less painful than you might think. In most cases, the medication and the injection are very well tolerated and there are no restrictions. If a bruise (hematoma) occurs, it can be a little painful for the first one or two days, after which sexual function can be resumed. It is important to know that the effect of such treatment occurs after 14-21 days at the earliest. The duration of the effect is about 4-6 months, after which the treatment can be repeated.

As with any injection, there may be a reaction to the botulinum toxin, but these are very rare. Infections are also a risk, but as with the face, they are extremely rare after such injections. The injection itself can lead to bruising, which is somewhat painful at first; this usually disappears within a few days at the latest.

Of course, successful treatment cannot be guaranteed in medicine, as medication can have different effects on different patients. Taking medication, pre-existing vascular diseases or other chronic illnesses can also impair the effect. If treatment for erectile dysfunction is requested, we recommend a urological examination beforehand.

So far, there is good evidence that botulinium toxin is effective on the penis and leads to the desired results.
